MEMBERSHIP CANCELLATION FORM

After a 6-month committment your membership can be cancelled.  Please remember to submit this form a minimum of 72 hours

prior to your next appointment (or monthly renewal) date.  Cancellations received in this time frame will be cancelled and no additional charges will be applied

to your credit card.   If you cancel within 24 hours of your scheduled appointment or renewal date there will be a 10% cancellation fee for a missed 

appointment; thereafter the account will be closed and no additional charges will occur.  Please see our general cancellation policy
